Birth Certificate Lost: How to request for Birth Certificate Copy

I Lost the Birth Certificate of my son who born in Qatar.   So, I follow the following procedure to get the new Birth Certificate copy.

1. Visit Ministry of Public Health (MoPH). On Google Map you can find the location. (Working time 7 am to 2 pm).
2. Take the token number.  Counters 7 and 8 are dealing with Birth Certificate and related matters.
3. Officer requested me to present the original documents. So following documents are required.

• Original passport of Baby, Father, and Mother
• Original QID of Baby, Father, and Mother
• I have also taken the health card of baby, but it was not requested. Recommending keeping it just in case.
• Note: No photocopies are requested so not required.   

4. After checking the record officer asked me to approach cashier counter and pay fee.
5. QAR 20 is fee per copy.
6. After paying the fee returned back the officer who I see at first instance and after showing the original payment receipt of QAR 20 receive the Birth Certificate copy.
