name change question

so i applied a visit visa for a relative during the fillup, i was forced to fillup all 5 name fields, so i did the following, for example

FIRST MIDDLE D E LEON

The last name is for example DE LEON, i separated D and E to fulfill the 5 fields. The visa got approved, but im just wondering if there will be no issue on departure and arrival.
